Client Services Agreement

 

Welcome to the Career Transitions Center of Chicago.  The success of your transition is dependent on the effort you commit to it.  The staff and volunteers of CTC are committed to you and your success. We will do our best to provide you with the training and support necessary so you can be at your best.

In order to access CTC services, each client understands and agrees to the following:

  1. To provide the services offered, CTC spends in cash and in-kind contributions more than $1000 per client.
  2. The CTC registration fee (your cost) is drastically reduced by donations from CTC alumni, faith communities, corporations, foundations, and individuals. Fees must be current to use the facilities of the Center and to participate in coaching or CTC Events and Programs. 
  3. In addition to paying your registration and renewal fees in a timely way, we ask, if you find CTC services valuable, to commit to supporting future clients of CTC by contributing to the organization once you have successfully completed your job search.
  4. Actively pursue your job search or career transition.  During the next three months complete CTC’s prescribed transitions program which includes attending four skills development workshops, participating in a weekly peer group and meeting regularly with a job coach.
  5. As there is great value in the relationships created among the clients, agree to help and network with other clients as your time permits.
  6. Agree to follow the CTC Code of Conduct (found on the CTC website).
  7. Cancel coaching appointments a minimum of 24 hours in advance (unless there is an emergency). Clients are also expected to arrive on time for appointments (and no later than 10 minutes). CTC reserves the right to cancel the client’s membership without refund if clients cancel two appointments with less than 24 hours notice or missed without notification or are late 2 or more times for appointments.
  8. Cancel  program attendance a minimum of 24 hours in advance (unless there is an emergency).
  9. For most, search or transition is completed within a year.  After one year, clients seeking to renew membership submit their progress plan and indicate their intention for continued use of the Center. Client petitions are evaluated by the Executive Director, along with staff and coaches, to ensure mutual suitability.
